The SS-5 Social Security form application must be filled in properly and submitted according to procedure. If not, it could cause severe delays. Although the form is relatively straightforward, the sheer scope of it can be daunting. Don’t worry, we will walk you through how to submit yours properly and get your card as quickly as possible.

Form SS-5 is the application for a Social Security card. Multipurpose, detailed, and subject to requirements, it is often a major hurdle to obtaining your new card. This document must be filled in according to your particular set of circumstances.
Whether it is due to changes in your life, theft, loss, or otherwise, you must fill out this form and submit it to the Social Security Administration along with supporting documentation if you wish to receive a new card.

Who needs an application for a Social Security card (form SS-5)?

The range of scenarios in which you may need this form is wide; however, the most common situations in which you may need an application for a Social Security card (form SS-5 + supporting documents) are:

  • Your First Card: If you have never had a card before, you may now need one for employment or school.
  • Marriage or Divorce: If you are recently married, it is imperative that you get a new SS card that reflects your name change. The same if you are getting divorced. This is especially important, but most couples do not think of it as a necessity.
  • Theft or Loss: If your card has been lost or stolen, you must first contact the FTC and monitor your credit report. Then, you must apply for a new card from the Social Security Administration.
  • To Fix Clerical Errors: You can use this Social Security application form to fix clerical errors such as the wrong birthdate, a name misspelling, or an error in the number.
  • For Children: If you have just given birth or have recently adopted, you will need a card to declare your child as a dependent on your taxes. Also, a Social Security card will help facilitate the opening of bank accounts in their name.

This is by no means an exhaustive list, but it covers the most common scenarios. If any of these describe your situation, you must fill out this form to obtain your card.

What information is needed for a Social Security application form?

Fill Social Security application form

As we mentioned previously, the form is not so complicated, though it can be exhaustive (and sometimes daunting). There is a great deal of information that must be covered. Be sure to fill the form in correctly the first time and check it several times, because, in the event of an error, it will be returned to you and may delay the process by weeks at a time.

In general, you must provide:

  • Your Name
  • Social Security Number
  • Place and Date of Birth
  • Citizenship Status
  • Ethnicity/Race (voluntary)
  • Sex
  • Mother/Father’s Name/Social Security Number

Aside from this, there are a few other requirements. Most of these are fairly straightforward.

Which documents are required for an application for a Social Security card

Passport As An Evidance Of Identity

Aside from your completed SS-5 form, you must submit documents to prove your identity, age, and citizenship (or immigration status).

According to the Social Security Administration’s website, these documents are acceptable:

Evidence of Age

  • Birth Certificate
  • Passport

In the event that you do not have a birth certificate, the administration will accept:

  • US Hospital Record of Your Birth
  • Religious Record Established Before the Age of 5
  • Final Adoption Decree

Evidence of Identity

  • US Driver’s License
  • US State-Issued Identity Card
  • US Passport

Non-US Citizens: If you are not a US-born citizen, you must prove your immigration status with documentation.

To prove your status, you may show:

  • Form I-551 (Lawful Permanent Resident Card, Machine Readable Immigrant Visa) with your unexpired foreign passport.
  • I-766 (Employment Authorization Document, EAD, work permit) or
  • I-94 (Arrival/Departure Record) or admission stamp in the unexpired foreign passport.

Please take note of this from the Social Security Administration (SSA)

The SSA informs all applicants that you must provide a properly completed application along with the proper supporting documents before processing can begin. They only accept original documents (not copies or notarized copies) or documents certified by the custodian of the original record. In the event that you have made a mistake, your application will not be processed.

Note: When applying for an original Social Security card, you must submit 2 supporting documents. If you are applying for a replacement, you only need to submit 1 document. If you wish to change information, you must submit documents to both prove your identity and to support the requested change (for example, a birth certificate to support a change of birth date).

You are limited to 3 in a calendar year or 10 over your lifetime, so don’t lose yours!
